System Safety

System safety is paramount in ensuring that all engineering solutions operate reliably and securely under all conditions. At AXIAH Ltd, we prioritize identifying and mitigating potential hazards to prevent accidents and ensure the well-being of users and operators. Our approach integrates safety considerations into every phase of the system lifecycle, from design to decommissioning.

AXIAH provides specialist System Safety support aligned with the Ministry of Defence (MOD) Acquisition Safety and Environmental Management System (ASEMS), DE&S Unified SEMS, Defence Safety Authority (DSA) regulations, and the requirements of Defence Standard (Def Stan) 00 056 (Safety Management Requirements for Defence Systems) and Def Stan 00 055 (Safety of Programmable Elements). Our role is to help customer organisations understand system level hazards, develop robust and defendable safety arguments, and collate proportionate evidence needed to demonstrate that risks have been reduced to As Low As Reasonably Practicable (ALARP).

We provide structured System Safety Management services that integrate with customer governance, engineering and programme management frameworks across the capability lifecycle. This includes hazard identification and analysis, risk assessment, safety requirements derivation, design and integration reviews, working groups, panels and committees. Where systems include software or complex hardware, we support compliance with programmable element expectations under Def Stan 00 055, including application of Safety Integrity Levels (SILs).

AXIAH develops and manages the artefacts required to support Safety Case development. This includes authoring Safety Management Plans (SMPs), maintaining compliant Hazard Logs, developing structured Safety Case arguments, producing Safety Case Reports (SCRs) for lifecycle milestones, and preparing Command level safety summaries to support Duty Holder and Senior Responsible Owner (SRO) decision making. Our support ensures that evidence is coherent, traceable and proportionate to the complexity and risk of the system, enabling credible progression through assurance, certification and Release to Service (RTS) activities.
